EMBOLISM AND RISKS FOLLOWING SURGERY

Thromboembolic disease is one of the most common serious complications following surgery. Embolism is a major danger facing many hospitalized patients today. The traumatic nature of the surgery, advanced age of the patients, prolonged bed rest and other factors, all contribute to the heightened level of risk. The incidence following major surgery in some groups may be as high as 50%; where multiple contributing factors are present, it may even be as high as 80%. Complicating matters further, is the availability of acceptable, proven effective methods of prophylaxis. Anticoagulant therapy may increase the risk of postoperative bleeding from the wound or other sites.

PATIENT ASSESSMENT

Deep vein thrombosis (DVT) is a far more common problem than is often realized. More than 2.5 million occur annually in the United States alone. Where due to age, obesity, previous history of thromboembolism or length and type of procedure, certain patients are at a greatly increased risk of developing DVT.

The National Institute of Health Consensus Report indicated effective DVT prophylaxis starts with assessment of patient risk factors, then subsequent selection of appropriate prophylactic regimen. The Vaso Press System offers a safe and effective noninvasive means of preventing DVT.

THE SOLUTION

The Vaso Press System is an external pneumatic compression (EPC) system for the prevention of DVT. Muscles are rendered ineffective as an assist in venous return during surgery. VasoPress functions as a secondary pump to propel the venous blood out of the deep veins while the patient is undergoing surgical procedures. The pump produces automatically timed cycles of compressed air that forces the blood out of the deep veins helping to prevent stasis. A burst of air is delivered to the garment that is wrapped around an extremity. The garments contain a bladder that focuses the compressed air on vascular structures in the leg. Since the effects of anesthesia and immobilization put the surgical patient at risk for thrombosis formation the resultant effect of moving blood out of the deep veins reduces the risk of DVT. A selection of garments is available depending upon the type of surgical procedure and the clinician’s preference.

VENOUS BLOOD MOVEMENT

Surgery changes the normal pathophysiology in the body with regard to both clot formation and fibrinolysis

ENHANCES FIBRINOLYSIS

Fibrinolytic shutdown occurs during surgery
Begins some time during surgery and may continue three to four days postoperatively
During this time, the risk of clot formation is extremely high
EPC reactivates the fibrinolytic system and reduces the risk of clot formation3

PATIENT COMPLIANCE

There are a variety of compression systems to choose. The descriptive terminology may be confusing from circumferential to sequential to intermittent to uniform. There are a lack of studies in peer-reviewed journals that definitively demonstrate that one form of compression is more effective than others to prevent DVT
